Why aren’t my forms complete?

Here are a few potential reasons:

  • After you submit your info to “Request Intake Forms,” we’ll email you in 3-5 business days with a link to complete your forms through Simple Practice, our client portal.

  • Potential Issue #1: This requires both individuals to finish forms in both profiles.

    Once you finish the (5)* forms in your individual profile, you must switch to your joint profile and finish the (3-4) forms there too. (Both people have 3 forms to complete in the joint profile, and the person paying for sessions has an additional credit card form.)

    *Addiction program consultations require more than 5 forms.

    Potential Issue #2: You’ve completed your forms, but your spouse/partner hasn’t completed their forms yet.

    Due to HIPAA privacy policies, we can’t update you on the status of your spouse/partner’s forms unless they list you on their Release of Information form. Both individuals must complete their own forms.

    Please have your spouse/partner reach out to us if you know you’ve completed your forms but you still haven’t received a booking email from our team.

  • Don’t worry; you won’t be charged until after you have a session with us. Also, your information is fully safe in our system since we can’t access your card information.

  • Our system doesn’t notify us when you submit the financial aid form. Please email us when you complete the form, so we know to review it.
