What happens next?
After submitting the “Request Intake Forms” information below, we’ll email you a link (within 3-5 business days) to complete your intake forms.
Once your forms are fully complete and we’ve reviewed them, you’ll receive another email with your assigned minister’s name and a link to book your session.
